Fix document examples for the correct usage

After I really tried out require-css, I found the document was a bit confused. This is the result of my struggling with all sorts of errors and confusion during optimization with r.js.

I'm not sure I understand this. So you are using map config AND paths then?

Why not just do:

map: {
  '*': {
    'css': 'my/path/to/require-css/css'
  }
}

??
